If you are planning to move to another state, we would like to help make the transition as smooth as possible. We recommend you let your provider know first via messages in the patient portal. Letting them know of your moving dates will be crucial to making sure appointments are set up so you don't run out of your medication. Then immediately reach out to the customer support team to inform us which state you are moving to.
We will check if there are available appointments with a provider in that state. If so, we will let you know and send you a medical release form for signature. After that is signed, a provider will review your medical records for a consideration of treatment. If accepted to their practice, we will help you book an initial appointment, which will be discounted to the follow-up pricing ($59).
*Please note you will need to fill out a new initial intake form and the new provider cannot prescribe medication prior to having that first visit.
If there are no available providers, we will send a blank medical release request. You will need to complete the form and let us know when it is finished. Then, we'll release your records to the secure patient portal after this has been received or they can be faxed to your new provider.
